What can be done to prevent it from ever happening again? After any type of incident, people are always greatly concerned and want to fix every distressing aspect. So where do you start? 1. Here are some areas to look at: - Training, operating procedures, maintenance and best practices - Safety audit - Automation, controls and alarms - Emergency response program ## Permanent fix To prevent future incidents, we’ve developed a unique approach, providing you with the corrective actions needed to resolve the issue permanently. ## Incident investigation When dealing with incidents, BBA performs an investigation that clearly demonstrates the damaging path and answers the question, “How did we get here?” The analysis lists all causes, including the root cause. ## Risk ranking The most difficult task is then to evaluate the factors that led to the incident and to select which risk-reducing actions to implement. For example, is it better to train employees or to invest in safety automation? The answer is unique to your plant; there is no “one-size-fits-all” textbook solution! ## The right recipe The level of effort required to reduce the risk of another serious event is determined by weighing gains from individual solutions and gains from combined actions. Which option will provide the most for the effort?
